Submit a Freedom of Information Request

You have a right to ask us what we do under the Freedom of Information Act (FOI).

It says we have to first acknowledge your enquiry and then supply the requested information within 20 working days.

There are some exemptions so please read our guidance to your right to know [pdf].

You can submit an FOI request by writing, phoning or emailing If you email us, please provide your telephone number so we can contact you if there is any query.

Before submitting an FOI request, please check our Model Publication Scheme to see if the information you require has been published on this website.

Because NHS Guildford and Waverley CCG is committed to open and transparent ways of working, you don't have to make a formal FOI request to ask us a question. Please get in touch using our contact us pages and we should reply sooner than 20 working days.
